I've let Yarn build the %HOME%\yarn\newsrc file for me -- that way, it
only includes newsgroups that I have actually viewed at some time (and
pseudo-newsgroups that I have specifically set up). When I unsubscribe
from a group, I eventually run 'rmgroup' in Yarn.

My newsgroup selection level list is only two screens long. It is
ordered with the groups I am most interested in at the top. Since I
can advance to the next unread group by hitting 'spacebar', it does
not bother me if newsgroup names with no unread messages still show.
